INCORRECT GRADE BOLTS WERE USED IN ATTACHING THE BODY TO THE CHASSIS AND IN ATTACHING THE SEAT ONTO THE CHASSIS. PROPER TORQUE COULD NOT BE ACHIEVED AND BODY COULDSEPARATE FROM THE CHASSIS DURING AN IMPACT.
